I am Larea, a Registered Nurse, Certified Dementia Specialist, and a Faith Community Nurse dedicated to helping you become your best caregiver and Patient Liaison!

New caregivers usually scour the internet to find information on care giving, but teaching caregivers the importance of monitoring the data needed by their doctor has been missing from these searches.

That is until now...

I have been a nurse for almost 30 years and have worked in nursing homes, hospice care, and assisted living facilities. In each of these areas, I monitored the data for the doctor and notified him of changes.

Every nurse out there knows this is the process, but caregivers at home do not!

I recognized that home caregivers were at a real disadvantage because no one shared this critical part of care giving.

In the past, the aging process was to send a loved one to a facility when they began to decline and became more challenging to take care of at home. So doctor's offices never thought about teaching caregivers at home what information they needed to collect and share with them.

Instead, the doctor's office assumed their patient would go to a nursing home at some point.

Due to rising nursing home costs, staffing shortages, and the loss of trust in facility care, more families are keeping their loved ones at home and taking care of them throughout the end of their life.

So we need to teach these caregivers how to fill that Patient Liaison role, catch minor issues, and report them to the doctor before they become severe or even life-threatening.

So, if you are a caregiver at home, I apologize somebody didn't identify this issue sooner! But, the good news is, I can help you solve this problem you didn't even realize you had.

I pulled from my years of experience and knowledge and created ALZlog. It is an app for your phone or computer to help you monitor the data you need to report to your doctor.

The app has clear, easy-to-use sections; the information can be downloaded and sent to your doctor's office. You can send weeks worth, six months, or even more. However long you have been recording the data, you can download and send it.

Not only that, but you can add family members and share what's going on with them. This app is excellent for long-distance family members who feel helpless and want to be more involved. If you have caregiver assistants coming into the home, the app is perfect for them (I had input from a home-helpers agency owner to help make it easier for them to use, so they should love this app).

If you are a frustrated, overwhelmed caregiver who needs a break, this app allows you to enter all the information a fill-in caregiver needs. So you can take a respite break and leave the worry behind, knowing they have the information they need, and you will be able to see the care given in the app (because we know you can't avoid taking a peek to make sure everything is ok).
